INTRODUCTION: Dementia cases are expected to rise to 81.1 million in 2040. Efforts are underway to develop diagnostic methods to facilitate early detection of the disease. Herein we review research findings focusing on pragmatic dysfunction in patients with dementia and evaluate the usefulness of assessing dementia and its progress with a battery of tests assessing figurative language skills. METHODS: A total of 74,778 article titles were identified from EMBASE, PubMed, and Google Scholar databases. After systematic screening, 51 journal articles were selected for the final review. RESULT: The review suggests that impaired figurative language might be a marker for early cognitive decline. Different forms of figurative language may be impaired at different stages of the disease and in different types of dementia involving different neuropathologies. CONCLUSION: The use of pragmatic tests in combination with the existing diagnostic protocols might increase the probability of early diagnosis. HIGHLIGHTS Pragmatic impairment could be a marker of early cognitive impairment. Figurative language-an important pragmatic aspect-is disrupted in mild cognitive impairment (MCI) and early Alzheimer's disease (AD). Figurative language impairment might precede literal language impairment. Pragmatic tests could be more sensitive than standard neuropsychological tests. Inclusion of pragmatic tests in diagnostic guidelines might bolster early detection.

BACKGROUND: Sensitive measures of cognition are needed in preclinical and prodromal Alzheimer's disease (AD) to track cognitive change and evaluate potential interventions. Neurofibrillary tangle pathology in AD is first observed in Brodmann Area 35 (BA35), the medial portion of the perirhinal cortex. The importance of the perirhinal cortex for semantic memory may explain early impairments of semantics in preclinical AD. Additionally, our research has tied figurative language impairment to neurodegenerative disease. OBJECTIVE: We aim to identify tasks that are sensitive to cognitive impairment in individuals with mild cognitive impairment (MCI), and that are sensitive to atrophy in BA35. METHODS: Individuals with MCI and cognitively normal participants (CN) were tested on productive and receptive experimental measures of semantic memory and experimental tests of figurative language comprehension (including metaphor and verbal analogy). Performance was related to structural imaging and standard neuropsychological assessment. RESULTS: On the experimental tests of semantics and figurative language, people with MCI performed worse than CN participants. The experimental semantic memory tasks are sensitive and specific; performance on the experimental semantic memory tasks related to medial temporal lobe structural integrity, including BA35, while standard neuropsychological assessments of semantic memory did not, demonstrating the sensitivity of these experimental measures. A visuo-spatial analogy task did not differentiate groups, confirming the specificity of semantic and figurative language tasks. CONCLUSION: These experimental measures appear sensitive to cognitive change and neurodegeneration early in the AD trajectory and may prove useful in tracking cognitive change in clinical trials aimed at early intervention.

Recent findings point to a role for hippocampus in the moment-by-moment processing of language, including the use and generation of semantic features in certain contexts. What role the hippocampus might play in the processing of semantic relations in spoken language comprehension, however, is unknown. Here we test patients with bilateral hippocampal damage and dense amnesia in order to examine the necessity of hippocampus for lexico-semantic mapping processes in spoken language understanding. In two visual-world eye-tracking experiments, we monitor eye movements to images that are semantically related to spoken words and sentences. We find no impairment in amnesia, relative to matched healthy comparison participants. These findings suggest, at least for close semantic links and simple language comprehension tasks, a lack of necessity for hippocampus in lexico-semantic mapping between spoken words and simple pictures.

Creative language is defined as linguistic output that is both novel and appropriate. Metaphors are one such example of creative language in which one concept is used to express another by highlighting relevant semantic features. While novelty is an inherent property of unfamiliar metaphors, appropriateness depends on the context. The current study tests the hypothesis that the context in which metaphors are encountered affects their processing. We examined the neural effects of comprehending metaphors in context by comparing neural activations in response to novel metaphors and literal sentences that were either embedded in a meaningful narrative or in matched jabberwocky contexts. We found that the neural correlates of processing metaphoric sentences and their literal counterparts are indistinguishable when embedded in a narrative: both conditions activate bilateral areas along the anterior temporal poles, middle temporal gyri, superior temporal sulci, and the angular gyri. Metaphors embedded in a narrative as compared to their identical counterparts embedded in jabberwocky show increased responses in sensorimotor areas that correspond to the modality of the literal meaning of the target word, perhaps reflecting deeper semantic processing. Our results confirm that context affects neural mechanisms for understanding creative ideas.

Despite the ubiquity of metaphor in cognition and communication, it is absent from standard clinical assessments of language, and the neural systems that support metaphor processing are debated. Previous research shows that patients with focal brain lesions can display selective impairments in processing metaphor, suggesting that figurative language abilities may be disproportionately vulnerable to brain injury. We hypothesized that metaphor processing is especially vulnerable to neurodegenerative disease, and that the left hemisphere is critical for normal metaphor processing. To evaluate these hypotheses, we tested metaphor comprehension in patients with left-hemisphere neurodegeneration, and in demographically matched healthy comparison participants. Stimuli consisted of moderately familiar metaphors and closely matched literal sentences sharing the same source term (e.g., The interview was a painful crawl / The infant's motion was a crawl). Written sentences were presented, followed by four modifier-noun answer choices (one target and three foils). Healthy controls, though reliably better at literal than metaphor trials, comprehended both sentence conditions well. By contrast, participants with left-hemisphere neurodegeneration performed disproportionately poorly on metaphor comprehension. Anatomical analyses show relationships between metaphor accuracy and patient atrophy in the left middle and superior temporal gyri, and the left inferior frontal gyrus, areas that have been implicated in supporting metaphor comprehension in previous imaging research. The behavioral results also suggest deficits of metaphor comprehension may be a sensitive measure of cognitive dysfunction in some forms of neurodegenerative disease.

Introduction: Relational memory is the ability to bind arbitrary relations between elements of experience into durable representations and the flexible expression of these representations. It is well known that individuals with traumatic brain injury (TBI) have declarative memory impairments, but less is known about how TBI affects relational memory binding, the deficit at the heart of declarative, or relational, memory impairment. The aim of the current study is to examine such deficits.Method: We used a spatial reconstruction task (SRT) with 29 individuals with TBI and 23 normal comparison (NC) participants to investigate four different types of spatial relations: (A) identity-location relations, i.e., the relationship between a specific item and its known location; (B) item-item relations, or the relationship between one item and another; (C) item-display relations, or the relationship between an item and its position in the display; and (D) compound-item relations, i.e., relations that involve combinations of A, B, and C.Results: Our data revealed that individuals with TBI showed impairments in learning identity-location relations and increased compound errors compared to NCs. We also found evidence that when item identity is disregarded, individuals with TBI do not perform differently from NCs. An exploratory analysis revealed that while relational memory performance was significantly correlated with scores on the California Verbal Learning Test (CVLT), more participants with TBI exhibited impairment on the SRT than of the CVLT.Conclusions: Our findings show that relational memory is impaired following TBI, and provide preliminary evidence for an easy-to-administer task with increased sensitivity to memory impairment.

We previously reported impoverished semantic memory in patients with hippocampal amnesia (Klooster & Duff, 2015). Here, we test whether this disruption results from the patients not updating semantic representations since the onset of their amnesia. We extend previous work by comparing performance of hippocampal patients and their current age (CA) comparisons (M = 58.5 years) to a new comparison group matched to the patients' age of onset (AoO) of hippocampal damage (M = 36.8). Participants completed feature and senses-listing tasks and the Word Associates Test. Both comparison groups performed significantly better than the patients with amnesia. A key new finding was that the older CA group performed significantly better than the younger AoO group. Semantic memory may become richer over time as additional information is added to existing representations. We conclude that a failure to update semantic memory may explain (at least some of) the previously observed deficits in amnesia and that the hippocampus may support semantic memory across the lifespan. Longitudinal data from patients with hippocampal pathology would provide a critical test of our conclusion.

Embodied cognition theories propose that the semantic representations engaged in during language comprehension are partly supported by perceptual and motor systems, via simulation. Activation in modality-specific regions of cortex is associated with the comprehension of literal language that describes the analogous modalities, but studies addressing the grounding of non-literal or figurative language, such as metaphors, have yielded mixed results. Differences in the psycholinguistic characteristics of sentence stimuli across studies have likely contributed to this lack of consensus. Furthermore, previous studies have been largely correlational, whilst patient studies are a critical way of determining if intact sensorimotor function is necessary to understand language drawing on sensorimotor information. We designed a battery of metaphorical and literal sentence stimuli using action and sound words, with an unprecedented level of control over critical psycholinguistic variables, to test hypotheses about the grounding of metaphorical language. In this Registered Report, we assessed the comprehension of these sentences in 41 patients with Parkinson's disease, who were predicted to be disproportionately affected by the action sentences relative to the sound sentences, and compared their performance to that of 39 healthy age-matched controls who were predicted to show no difference in performance due to sensory modality. Using preregistered Bayesian model comparison methods, we found that PD patients' comprehension of literal action sentences was not impaired, while there was some evidence for a slowing of responses to action metaphors. Follow up exploratory analyses suggest that this response time modality effect was driven by one type of metaphor (predicate) and was absent in another (nominal), despite the fact that the action semantics were similar in both syntactic forms. These results suggest that the conditions under which PD patients demonstrate hypothesized embodiment effects are limited. We offer a critical assessment of the PD action language literature and discuss implications for the embodiment debate. In addition, we suggest how future studies could leverage Bayesian statistical methods to provide more convincing evidence for or against embodied cognition effects.

The impact of traumatic brain injury (TBI) on procedural memory has received significantly less attention than declarative memory. Although to date studies on procedural memory have yielded mixed findings, many rehabilitation protocols (e.g., errorless learning) rely on the procedural memory system, and assume that it is relatively intact. The aim of the current study was to determine whether individuals with TBI are impaired on a task of procedural memory as a group, and to examine the presence of individual differences in performance. We administered to a sample of 36 individuals with moderate-severe TBI and 40 healthy comparisons (HCs) the rotary pursuit task, and then examined their rate of learning, as well as their retention of learning. Our analyses revealed that while individuals with TBI spent a significantly shorter amount of time on target as a group, they did not retain significantly less procedural learning, and as a group their rate of learning was not different from HCs. However, there were high individual differences in both groups, indicating that some individuals might not be able to take advantage of treatment methods designed to leverage intact procedural memory system. Future work is needed to better assess and characterize procedural memory in individuals with TBI across a larger battery of tasks in experimental and clinical setting as memory and learning status may predict rehabilitation success.

The necessity of the hippocampus for acquiring new semantic concepts is a topic of considerable debate. However, it is generally accepted that any role the hippocampus plays in semantic memory is time limited and that previously acquired information becomes independent of the hippocampus over time. This view, along with intact naming and word-definition matching performance in amnesia, has led to the notion that remote semantic memory is intact in patients with hippocampal amnesia. Motivated by perspectives of word learning as a protracted process where additional features and senses of a word are added over time, and by recent discoveries about the time course of hippocampal contributions to on-line relational processing, reconsolidation, and the flexible integration of information, we revisit the notion that remote semantic memory is intact in amnesia. Using measures of semantic richness and vocabulary depth from psycholinguistics and first and second language-learning studies, we examined how much information is associated with previously acquired, highly familiar words in a group of patients with bilateral hippocampal damage and amnesia. Relative to healthy demographically matched comparison participants and a group of brain-damaged comparison participants, the patients with hippocampal amnesia performed significantly worse on both productive and receptive measures of vocabulary depth and semantic richness. These findings suggest that remote semantic memory is impoverished in patients with hippocampal amnesia and that the hippocampus may play a role in the maintenance and updating of semantic memory beyond its initial acquisition.

Hand gesture, a ubiquitous feature of human interaction, facilitates communication. Gesture also facilitates new learning, benefiting speakers and listeners alike. Thus, gestures must impact cognition beyond simply supporting the expression of already-formed ideas. However, the cognitive and neural mechanisms supporting the effects of gesture on learning and memory are largely unknown. We hypothesized that gesture's ability to drive new learning is supported by procedural memory and that procedural memory deficits will disrupt gesture production and comprehension. We tested this proposal in patients with intact declarative memory, but impaired procedural memory as a consequence of Parkinson's disease (PD), and healthy comparison participants with intact declarative and procedural memory. In separate experiments, we manipulated the gestures participants saw and produced in a Tower of Hanoi (TOH) paradigm. In the first experiment, participants solved the task either on a physical board, requiring high arching movements to manipulate the discs from peg to peg, or on a computer, requiring only flat, sideways movements of the mouse. When explaining the task, healthy participants with intact procedural memory displayed evidence of their previous experience in their gestures, producing higher, more arching hand gestures after solving on a physical board, and smaller, flatter gestures after solving on a computer. In the second experiment, healthy participants who saw high arching hand gestures in an explanation prior to solving the task subsequently moved the mouse with significantly higher curvature than those who saw smaller, flatter gestures prior to solving the task. These patterns were absent in both gesture production and comprehension experiments in patients with procedural memory impairment. These findings suggest that the procedural memory system supports the ability of gesture to drive new learning.
